1. We will set a physical therapy plan with goals for each patient.

2. Physical therapy notes will be delivered to the doctor before the patient’s next doctor visit.

3. The patient will be scheduled for physical therapy within 48 hours of referral from the doctor.

4. We will schedule appointments from 7:00 a.m. to 5:15 p.m. to accommodate patient’s schedule.

5. Upon arrival, patient wait time will be less than 15 minutes.
